Wegovy (semaglutide) is a prescription GLP-1 for weight loss that also lowers heart-attack and stroke risk. It now comes three ways:
- A once-weekly pen: 2.4 mg maintenance, 14.9% average weight loss in its main trial
- A higher-dose Wegovy HD 7.2 mg pen
- A once-daily pill: 25 mg maintenance, 13.6% average weight loss
Without insurance, Novo Nordisk sells the pill from $149 and the pen at $199 for the first two months, then $349 a month (list price is about $1,349). Covered commercial insurance plus the savings card can mean $25, and eligible Medicare members pay $50 through the GLP-1 Bridge. There's no generic.
Wegovy pill vs. Wegovy pen
Wegovy pill
- Doses
- 1.5 mg → 4 mg → 9 mg → 25 mg maintenance (30 days per step)
- Weight loss
- 13.6% average at 25 mg over 64 weeks (2.4% on placebo)
- How to take
- Every morning, empty stomach, up to 4 oz of water, then wait 30 minutes before food, drinks or other pills
- Cash price
- $149 → $199 → $299 a month
- Approved for
- Weight loss and heart-risk reduction in adults
Wegovy pen
- Doses
- 0.25 → 0.5 → 1 → 1.7 → 2.4 mg maintenance (4 weeks per step), optional 7.2 mg Wegovy HD
- Weight loss
- 14.9% at 2.4 mg over 68 weeks; 18.8% at 7.2 mg over 72 weeks
- How to take
- Same day each week, any time, with or without food
- Cash price
- $199 for the first 2 months, then $349 (HD: $399)
- Approved for
- Weight loss (adults and teens 12+), heart-risk reduction, and MASH liver disease
- Choose the pill if you'd rather avoid injections and can reliably take it fasting each morning. It's also the cheapest way to start ($149).
- Choose the pen if you want once-a-week dosing with no food rules, you're under 18, or you may need the 7.2 mg dose for more weight loss.
- You can switch: the label allows moving from the 2.4 mg pen to the 25 mg pill (start one week after the last injection), or from the pill back to the pen (start the day after the last tablet).
What Wegovy costs, by how you pay
Sources: Novo Nordisk Wegovy savings and NovoCare pricing; CMS Medicare GLP-1 Bridge fact sheet; CMS NADAC; TrumpRx.gov. Prices change; we confirm yours when you call.
- The "$25" fine print. The savings card covers at most $100 of your copay each month. A $300 copay becomes $200, not $25. A month is one box of 4 pens or one bottle of 30 tablets.
- Self-pay goes through NovoCare. Novo Nordisk's $149–$399 cash prices are filled by NovoCare Pharmacy with home delivery. GoodRx and TrumpRx list the same Novo cash prices. They aren't a separate, deeper discount. See how all the GLP-1 programs compare.
- Medicaid in Connecticut: HUSKY stopped covering GLP-1s prescribed only for weight loss in mid-2025. Wegovy may still be covered for other approved uses, such as heart-risk reduction, usually with prior authorization. Manufacturer cards can't be used with Medicaid.

Doses and dosing schedules
Wegovy pen (weekly):
0.25 mg, a starting dose to let your stomach adjust
0.5 mg
1 mg
1.7 mg
2.4 mg maintenance (1.7 mg if 2.4 mg isn't tolerated)
7.2 mg (Wegovy HD) for adults who've tolerated 2.4 mg for at least 4 weeks and need more weight loss
Wegovy pill (daily): 1.5 mg for days 1–30, 4 mg for days 31–60, 9 mg for days 61–90, then 25 mg from day 91.
Take it first thing in the morning on an empty stomach with no more than 4 ounces of plain water. Swallow it whole, then wait at least 30 minutes before eating, drinking anything else (including coffee) or taking other pills. Food and extra liquid reduce how much semaglutide you absorb.
- Missed pen dose: if your next dose is more than 2 days away, take it as soon as you can. If it's less than 2 days away, skip it. If you miss 2 or more weeks in a row, your prescriber may restart you at a lower dose.
- Missed pill: skip it and take the next one the following day.
- Pen options: single-dose pens, single-dose syringes, and the Wegovy FlexTouch pen, which holds four 2.4 mg doses. Never share a FlexTouch pen, even with a new needle.
Results: weight, heart and liver
Source: Wegovy prescribing information (June 2026), Clinical Studies. Includes people who stopped treatment early. In the 7.2 mg trial, 63.8% of people on 7.2 mg lost at least 15% of body weight, vs. 7.5% on placebo. Bars scaled to 18.8%.
SELECT trial
- Who
- 17,604 adults with established heart disease and overweight or obesity, without diabetes
- Result
- Heart death, heart attack or stroke: 6.5% on Wegovy vs. 8.0% on placebo, a 20% lower risk
- Why it matters for cost
- This approval lets Medicare Part D and many plans cover Wegovy outside the weight-loss exclusion
MASH (fatty liver)
- Approval
- Accelerated FDA approval for noncirrhotic MASH with moderate to advanced (F2–F3) liver fibrosis
- Dose
- 2.4 mg weekly pen
- Note
- Continued approval depends on a confirmatory outcomes trial

In the only head-to-head trial of the injections (SURMOUNT-5), tirzepatide averaged 20.2% weight loss versus 13.7% for semaglutide. Wegovy's advantages are its pill option, its heart-risk approval and the lower cash price.
Side effects
Bars scaled to 44%. At 7.2 mg, nausea was reported in 39%, vomiting in 22%, hair loss in 6%, and unusual skin sensations (tingling, burning, skin sensitivity) in 22%, vs. 6% at 2.4 mg.
Manage at home
- Nausea and fullness: smaller meals, stop eating when full, avoid greasy food
- Constipation: fluids and fiber
- Most stomach effects ease after each dose step
Report soon
- Vomiting or diarrhea that won't stop (dehydration can harm the kidneys)
- A racing heartbeat at rest
- Vision changes if you have type 2 diabetes
Urgent
- Severe stomach pain going to the back (pancreatitis)
- Upper-right belly pain, fever or yellow skin (gallbladder)
- Swelling of the face or throat, or trouble breathing
- A neck lump, hoarseness or trouble swallowing
Warnings and who shouldn't use it
Semaglutide caused thyroid C-cell tumors in rodents. It's unknown whether it does in people. Don't use Wegovy if you or a family member has had medullary thyroid carcinoma (MTC), or if you have MEN 2.
- Don't combine with Ozempic, Rybelsus, Zepbound, Mounjaro or any other GLP-1 or tirzepatide product, including compounded versions
- Pregnancy: stop at least 2 months before a planned pregnancy, because semaglutide stays in the body for weeks
- Surgery or sedation: tell every clinician you take Wegovy, because a slowed stomach raises the risk of inhaling food during anesthesia
- Insulin or sulfonylureas: doses may need lowering to avoid low blood sugar
- Type 2 diabetes with eye disease: rapid blood-sugar improvement has been linked to worsening diabetic retinopathy, so keep your eye exams

Frequently asked questions
How much does the Wegovy pill cost?
Without insurance, Novo Nordisk's NovoCare self-pay price is $149 a month for the 1.5 mg starting dose, $199 for 4 mg and $299 for the 9 mg and 25 mg doses. With commercial insurance that covers it, the Wegovy savings offer can bring it to as little as $25 a month (it takes off at most $100 a month). The list price is about $1,349 a month. Eligible Medicare members can get it for $50 a month through the GLP-1 Bridge.
Is the Wegovy pill as effective as the injection?
They are close. In its 64-week FDA trial, the 25 mg Wegovy pill produced 13.6% average weight loss versus 2.4% on placebo. The 2.4 mg pen produced 14.9% versus 2.4% in its 68-week trial. These were different trials, not a head-to-head comparison. The label notes that blood levels with the 25 mg pill are predicted to be comparable to the 2.4 mg injection, but more variable. The pill must be taken on an empty stomach with a small amount of water, which matters for how well it works.
Is Wegovy the same as Ozempic?
Both contain semaglutide and are made by Novo Nordisk, but they're approved for different uses and doses. Wegovy is approved for weight loss (up to 2.4 mg weekly, or 7.2 mg with Wegovy HD), heart-risk reduction and a form of fatty liver disease (MASH). Ozempic is approved for type 2 diabetes (up to 2 mg weekly). Insurers cover them under different rules, and they should never be used together.
How do I get Wegovy for $25?
You need commercial insurance that covers Wegovy, a prescription for an approved use, and the Wegovy savings offer from Novo Nordisk. It works for both the pen and the pill. You pay as little as $25 a month, but the card takes off at most $100 a month, so $25 only works if your plan's copay is about $125 or less. It can't be used with Medicare, Medicaid or other government insurance.
Does Medicare cover Wegovy?
Yes, in two ways. Part D plans can cover Wegovy to reduce heart-attack and stroke risk in people with established heart disease and overweight or obesity. For weight loss, the Medicare GLP-1 Bridge (July 1, 2026 to December 31, 2027) lets eligible Part D members get any dose of the Wegovy pen or pill for $50 a month.
Can I send a Wegovy self-pay prescription to a regular pharmacy?
Novo Nordisk's own self-pay prices ($149–$399) are filled through NovoCare Pharmacy with home delivery. At a regular pharmacy, a cash purchase without a program runs close to the list price. If you have insurance that covers Wegovy, a local pharmacy fill with the $25 savings card is usually cheapest. Call us and we'll tell you which route is cheaper for you.
What are the Wegovy doses?
Pen: start at 0.25 mg weekly, then 0.5 mg, 1 mg and 1.7 mg, each for 4 weeks, reaching the 2.4 mg maintenance dose at week 17. Adults who tolerate 2.4 mg for at least 4 weeks and need more weight loss can go up to 7.2 mg (Wegovy HD). Pill: 1.5 mg daily for 30 days, then 4 mg and 9 mg for 30 days each, then 25 mg daily from day 91.
How do I take the Wegovy pill?
Take it once a day in the morning on an empty stomach, with up to 4 ounces (half a cup) of plain water. Swallow it whole. Then wait at least 30 minutes before eating, drinking anything else or taking other pills. If you miss a dose, skip it and take the next one the following day.
What are the most common side effects of Wegovy?
With the 2.4 mg pen, the most common were nausea (44% vs. 16% on placebo), diarrhea (30% vs. 16%), vomiting (24% vs. 6%), constipation (24% vs. 11%), stomach pain (20% vs. 10%), headache, fatigue, indigestion and dizziness. Hair loss occurred in 3% (vs. 1%). At 7.2 mg, unusual skin sensations such as tingling or burning were reported in 22% of people.
Is there a generic Wegovy?
No. Semaglutide is still under patent in the US and there is no FDA-approved generic. Compounded "semaglutide" is not a generic. The FDA declared the semaglutide shortage over in early 2025, which ended the temporary allowance for mass compounding of copies.
Does Wegovy cause hair loss?
Hair loss was reported in 3% of people on the 2.4 mg pen versus 1% on placebo, and in 6% of people on the 7.2 mg dose. Rapid weight loss itself can trigger temporary shedding. Tell your prescriber if you notice it.
Wegovy 25 mg pill vs. Zepbound: which works better?
There's no head-to-head trial of the Wegovy pill against Zepbound. In their FDA trials, the 25 mg Wegovy pill averaged 13.6% weight loss over 64 weeks and Zepbound 15 mg averaged 20.9% over 72 weeks. In the one head-to-head injection trial (SURMOUNT-5), tirzepatide (Zepbound) averaged 20.2% versus 13.7% for semaglutide (Wegovy). The pill costs less in cash ($299 vs. $449 a month at maintenance) and needs no injections.
Can I drink alcohol on Wegovy?
The label doesn't list alcohol as a drug interaction. But alcohol can worsen nausea and dehydration, and adds to the risk of low blood sugar if you also take insulin or a sulfonylurea. Heavy drinking is a risk factor for pancreatitis, one of Wegovy's warnings. Moderation, or avoiding it during dose increases, is sensible.
When should I stop Wegovy before pregnancy?
The label says to stop Wegovy at least 2 months before a planned pregnancy, because semaglutide stays in the body for weeks. If you become pregnant while taking it, contact your prescriber.
